Good question. I'm currently thinking of ways to spread the word about CM in a smaller-sized city that is already somewhat bike-accomodating (there are a few painted bike lanes in place), but could use more attitude adjustment on the part of the drivers...
Putting a listing in the 'events' section of the local alt-weekly or arts 'n culture section of a major paper might be one way, as well as in the local university paper (if there is one in yer town)...

if there's anything similar to santa cruz's bike church or SF's bike kitchen, hit 'em up. Don't just put up a flier- talk to the mechanics, make sure they're interested and will spread the word.
Making a set meeting place and time is very important, so that fliers stay useful.
The first or second one may be small, but they'll grow, so no worries.

Make a bunch of cool flyers ... yes. Give them to every cyclists you can.
List your CM online whereever you can. It will be very slow at first but
keep doing it. During the first mass hand out flyers to people in the street.
Even if's just you and a few friends. People love a parade.
Tip off the media that you're doing (anonymously)
Don't tell the cops.
